Update the approval breaker (per-session refusal threshold + window)

Configure when an agent's session stops asking for approval. When a session's approval requests are refused (denied, or expired or cancelled with no decision) at least refusals_threshold times within window_seconds, no new approval request is raised for it: the call is denied and the session told to stop. A routine run ends, a chat turn ends with the explanation, and an external client gets it in the tool result. The breaker closes once refusals age out of the window.
